Shifts are 7-3, 3-11, 11-7 (rotating weekends) FT and PT Options.
Essential Duties and Responsibilities include, but are not limited to:
· Follow the service plan in providing services to residents.
· Maintain confidentiality and HIPAA regulations in accordance with facility policies and federal regulations.
· Assist residents with activities of daily living, including bathing, dressing, eating, toileting, hygiene and mobility.
· Encourage and promote socialization among residents.
· Assist with the activity program; conduct activities as directed by the Activity Director or assigned by supervisor.
· Encourage and/or remind residents to participate in activities. Promote activities enthusiastically to pique residents' interest.
· Chaperone activities outside the facility when requested.
· Assist with monitoring health, safety and well-being of residents.
· Document information pertaining to changes in residents' functional status, personal needs, appointments, etc.
· Assist in keeping the environment safe and clean for residents.
· Provide one-on-one support/visitation to assigned residents as needed.
· Report concerns about residents to supervisor and coworkers.
· Respond to resident emergencies; provide first-aid assistance and arrange for appropriate medical attention as needed.
· Identify supplies needed to perform tasks and report needs to supervisor.
· Maintain an accurate record of chargeable services (transportation, cleaning, personal laundry. guest meals, guest room use, etc.). Submit record to Resident Care Director by the designated date each month.
· Practice safety in working and with equipment, chemicals, tools and utensils.
· Participate in meetings to discuss needs and requirements of each resident and takes appropriate action as needed and directed to promote quality resident care.
· Assist in the development and updating of each resident's service plan.
· Assist staff in providing and coordinating services to residents.
· Encourage socialization among residents.
· Ensure excellent customer service.
· Assist in completion of required documentation to meet federal/ state and facility guidelines.
· Role models geriatric care techniques to staff and assist other staff as needed.
· Complete all required competencies.
· Work schedule as assigned.
· Ensure resident safety.
Other duties include, but are not limited to:
· Do personal laundry for residents if needed.
· Strip and make residents' beds weekly as needed.
· Assist in preparing meals, serving meals and bussing tables during mealtimes if necessary.
· Provide transportation services, using company vehicle when requested.
· Participates in Quality/Process Improvement programs as requested.
· Carry out other duties as assigned.
· Assist with Emergency Plan implementation as assigned.
· Maintenance: changing light bulbs, batteries in smoke detectors and thermostats, clearing vehicles of snow/ice, light snow shoveling/ clearing walk ways, etc.
· Support marketing events as needed.
· Daily light housekeeping and/or minimal animal care.
· Assist with the activity program; conduct activities as directed.
· Perform blood glucose testing via glucometer on residents as directed (after competency testing).
· Obtaining proper vital signs on residents as directed/ scheduled (after competency testing).
· Assist residents with oxygen, ostomies, and other medical devices per scope of practice and facility protocol (after competency testing
