Responsibilities:

The Resident Care Coordinator is responsible for assisting residents with accessing services both inside Oakey Assisted Living & Memory Care and in the greater community.

•Acts as the community liaison and provides assistance to residents and families during move-ins and transitions between levels of living and assists residents and families during times of need , providing general support.

•Accurately communicates benefits of living at Oakey Assisted Living & Memory Care.

•Assists residents and families during times of need (e.g. end of life, providing general support, etc.)

•Provides emotional support during moments of grief and loss.

•Provides guidance and assistance with obtaining and maintaining health and emergency information for residents.

*Assists in evaluating and drafting care plans for residents.

*Reports directly to the Memory Care & Assisted Living Director.

Skills:
- At least one year experience as a Resident Care Coordinator or similar position.
- Knowledge of direct support practices in various settings.
- Ability to provide compassionate care and emotional support to clients.
- Strong communication skills to interact effectively with clients, families, and healthcare professionals.
- Proficient in coordinating care services and managing multiple tasks efficiently.

Job Types: Full-time, Part-time

Pay: $17.00 - $19.00 per hour

Expected hours: No less than 40 per week
