Overview:
Reliatus Behavioral Health is seeking a dedicated and experienced Manager of Case Management to join our team in St. Paul, Minnesota. The ideal candidate will possess strong leadership skills, a passion for helping others, and the ability to effectively manage and support our case management team. This role requires a Bachelor's Degree in Human Services, Social Work, Psychology, Sociology, or a related field.
Key Responsibilities:
- Oversee the daily operations of the case management department, ensuring high-quality service delivery.
- Supervise and support case managers, providing guidance and professional development opportunities.
- Develop and implement case management policies and procedures to enhance service efficiency and effectiveness.
- Ensure compliance with all relevant regulations and standards in case management practices.
- Manage and resolve complex cases and issues, providing expertise and direction as needed.
- Coordinate with other departments and external agencies to facilitate comprehensive care for clients.
- Monitor and evaluate program performance, using data to inform improvements and innovations.
- Prepare and present reports on case management activities and outcomes to senior management.
Qualifications
- Bachelor’s Degree in Human Services, Social Work, Psychology, Sociology, or a related field.
(Social Workers must be licensed in the State of MN)
-Must have experience working in waivered programs under the Minnesota Department of health services waiver program.
- Proven experience in case management, with a minimum of 3-5 years in a supervisory or managerial role.
- Strong understanding of case management principles, practices, and regulations.
- Excellent leadership, organizational, and communication skills.
- Ability to work effectively in a fast-paced and dynamic environment.
- Proficient in using case management software and other relevant technologies.
