Bereavement Coordinator jobs in Kentucky
Bereavement Coordinator plans and provides bereavement counseling and support to survivors. Assesses individual needs and coordinates with multi-disciplinary teams to develop and deliver a holistic support plan. Being a Bereavement Coordinator guides survivors to appropriate bereavement resources. Facilitates grief support group sessions. Additionally, Bereavement Coordinator requires a bachelor's degree in social work. May require the applicable State License to Practice Clinical Social Work. Typically reports to a manager. The Bereavement Coordinator occasionally directed in several aspects of the work. Gaining exposure to some of the complex tasks within the job function. Bereavement Counselor
Location: Lexington, KY
Schedule: Monday-Friday 8am-5pm with occasional evenings & weekends; Position will require team member to work in office daily upon hire, during a 6-month introductory period. After introductory period is complete, team member may be eligible to work remotely.
As a BCN Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness, but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

A team member in this position provides counseling and supportive services to help meet patient/family and community social and emotional needs in relation to the impact of a terminal illness and loss and grief. The Bereavement Counselor will work under the direction of the Executive Director and/or the Regional Counseling Supervisor.
Requirements- Master's or Bachelor's degree in counseling, psychology, nursing, social work, family studies or other mental health discipline, certified/licensed per state law
- Minimum of three years supervised experience in health or related field
- Clinical experience in related field
- Ability to exercise judgment and discretion in confidential matters
- Knowledge and commitment to Bluegrass Care Navigators’ philosophy
- Understanding of grief response and experience with counseling individuals and/or within groups regarding loss.
- Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
- Must be a licensed driver with automobile that is insured in accordance with state and/or BCN requirements and is in good working order.
