Bereavement Coordinator plans and provides bereavement counseling and support to survivors. Assesses individual needs and coordinates with multi-disciplinary teams to develop and deliver a holistic support plan. Being a Bereavement Coordinator guides survivors to appropriate bereavement resources. Facilitates grief support group sessions. Additionally, Bereavement Coordinator requires a bachelor's degree in social work. May require the applicable State License to Practice Clinical Social Work. Typically reports to a manager. We're seeking candidates with post-training experience in therapy, eligibility for licensure, and a background in working with traumatized children and / or adolescents.
This full-time position involves traveling an average of 8 hours weekly throughout Vermont to family’s home, with flexible scheduling.
Responsibilities include assisting youth and family overcome obstacles, coordination of treatment, discharge planning, case management, community integration, documentation, and active participation in weekly team meetings.
