Case Management Director oversees a staff of case managers responsible for patient care coordination. Develops and implements case management programs, including utilization review, intake or discharge planning, and managed care contracting or negotiation. Being a Case Management Director evaluates patient care data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Seeks treatments that balance clinical and financial concerns with the family's needs and the patient's quality of life. Responsibilities:
- Conduct comprehensive assessments of patients' medical, social, and psychological needs
- Develop individualized care plans in collaboration with the interdisciplinary team
- Coordinate and facilitate communication between patients, families, and healthcare providers
- Provide counseling and support to patients and their families
- Monitor patient progress and adjust care plans as necessary
- Collaborate with community resources to ensure continuity of care
- Advocate for patients' rights and ensure they receive appropriate services
- Document all interactions and interventions accurately and timely
Requirements:
- Bachelor's degree in social work or related field
- Experience in case management or related field (6 months paid experience required)
- Knowledge of medical terminology and healthcare systems
- Familiarity with IDD provider settings
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
- Proficiency in managed care principles and practices
- Understanding of mental health conditions and referral techniques
- Knowledge of discharge planning processes
- Ability to handle stressful situations with empathy and professionalism
