Case Manager - Alcohol or Drug coordinates the overall plan of care for patients with alcohol or drug problems from admission to discharge. Acts as a liaison between patient/family and service delivery personnel to ensure patients are effectively evaluated and treated. Being a Case Manager - Alcohol or Drug works with management to maintain and improve overall quality of services. Typically requires a bachelor's degree in a related field. Additionally, Case Manager - Alcohol or Drug requires a valid Licensed Chemical Dependency Counselor(LCDC) license. Typically reports to a manager or head of a unit/department. The Case Manager - Alcohol or Drug occasionally directed in several aspects of the work. Gaining exposure to some of the complex tasks within the job function. Job Objective: Provide substance use disorder treatment/counseling/case management for agency clients and families, including adolescents - in english and spanish.
Job Responsibilities: Provide counseling, crisis intervention, referrals, documentation, discharge planning, treatment planning, monthly statistical reporting, participate in clinical and administrative supervisions, function as part of interdisciplinary treatment team. Maintain specified client caseload. Be versed in agency policy and procedure regarding clients and collateral agencies. Provide individual, family, and group counseling as necessary. Perform intake/assessment/evaluation as needed. Provide all other job-related activities at the discretion of the Program Director.
Basic Requirements: CASAC with bachelor’s degree, or Licensed Masters-level degree in Social Work or Mental Health Counseling, with CASAC preferred. Experience in addiction counseling preferred. Availability Monday through Friday and must include evenings.
Critical Skills/Expertise: Counseling skills, including evidenced-based practice model knowledge, clinical assessment, case recording, customer service proficiency, liaison skills with legal and other agencies. Professional presentation of self and representation of agency.
