Acts as the liaison between patient/family and homecare personnel to ensure necessary care is provided promptly and effectively. Being a Case Manager - Home Care/Hospice requires a bachelor's degree. Typically reports to a manager. Additionally, Case Manager - Home Care/Hospice requires RN license. Case Manager - Home Care/Hospice's years of experience requirement may be unspecified.
