Case Manager coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). Case Manager's years of experience requirement may be unspecified. Responsibilities:
- Conduct comprehensive assessments of patients' medical, social, and psychological needs
- Develop individualized care plans in collaboration with the interdisciplinary team
- Coordinate and facilitate communication between patients, families, and healthcare providers
- Provide counseling and support to patients and their families
- Monitor patient progress and adjust care plans as necessary
- Collaborate with community resources to ensure continuity of care
- Advocate for patients' rights and ensure they receive appropriate services
- Document all interactions and interventions accurately and timely
Requirements:
- Bachelor's degree in social work or related field
- Experience in case management or related field (6 months paid experience required)
- Knowledge of medical terminology and healthcare systems
- Familiarity with IDD provider settings
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
- Proficiency in managed care principles and practices
- Understanding of mental health conditions and referral techniques
- Knowledge of discharge planning processes
- Ability to handle stressful situations with empathy and professionalism
