What the position looks like:
This position will direct the clinical aspects of home health and hospice care provided in the home setting, and within the aging population. Provide oversight to a skilled team in areas such as wound care, IV/ infusions, cardiopulmonary assessment, diabetic teaching, post-surgical care, disease management, medication reconciliation and management, gastrostomy tube care, injections, blood draws, in-dwelling catheter care and management; and provide comfort care and education around disease process to patients and their families.
Foster a framework that allows all team members to flourish in their field. Assist the team with the development of client-centered, individualized plan of care to meet the needs of the patient and family. Drive quality improvement initiatives, safety and infection control initiatives, and so much more.
