This well-respected 60 bed non-profit hospital is a center of high patient care and excellence. Reporting to the Director of Case Management and the Chief Nursing Officer, the Clinical Case Manager leads department and professional development initiatives.

The RN Case Manager will oversee the coordination of care delivery, gather data for quality assessments, and serve as a liaison with physicians. Key responsibilities include assessing patient discharge needs, tracking patient progress, and working closely with the health care team. This role also involves communicating with third-party payers, performing case reviews, and developing comprehensive discharge plans. Additionally, the RN Case Manager will ensure the timely creation of care plans, monitor health care processes, and engage in performance improvement activities. Staying current with industry knowledge and ensuring the satisfaction of both patients and staff are crucial aspects of this position.

The Clinical Case Manager works closely with a diverse group of hospital staff to ensure comprehensive patient care. This includes collaborating with physicians to develop and implement individualized care plans and coordinating with nurses to monitor patient progress and address any immediate concerns. The RN Case Manager also will collaborate with social workers to facilitate smooth patient transitions and discharge planning, as well as with physical and occupational therapists to integrate rehabilitation services into patient care. Additionally, they communicate with pharmacists to ensure accurate medication management and work with administrative staff to handle the necessary documentation and communication with third-party payers. This multidisciplinary approach ensures that all aspects of a patient’s health care needs are addressed efficiently and effectively and top of the line at this facility!

The Clinical Case Manager is expected to have knowledge on the principles and practices of Nursing Education and Patient Care. This person will be responsible for serving as a role model, and initiator in problem-solving, decision-making processes, delegating, and communicating information to the nursing staff.
