The RN Case Manager will oversee the coordination of care delivery, gather data for quality assessments, and serve as a liaison with physicians. Key responsibilities include assessing patient discharge needs, tracking patient progress, and working closely with the health care team. This role also involves communicating with third-party payers, performing case reviews, and developing comprehensive discharge plans. Additionally, the RN Case Manager will ensure the timely creation of care plans, monitor health care processes, and engage in performance improvement activities. Staying current with industry knowledge and ensuring the satisfaction of both patients and staff are crucial aspects of this position.
The Clinical Case Manager works closely with a diverse group of hospital staff to ensure comprehensive patient care. This includes collaborating with physicians to develop and implement individualized care plans and coordinating with nurses to monitor patient progress and address any immediate concerns.
