Partners In Care seeks a Community Relations Specialist to establish and maintain relationships with organizations and individuals utilizing our home health, hospice, palliative care, and transitions services. This role involves educating referral sources, such as facilities, medical clinics, and community groups, and promoting open communication about our services.
Compensation: $32-$42 per hour (DOE).
Location: Bend, Oregon.
Schedule: Full-time (40 hours), Monday- Friday.

What You Get To Do:
- Optimize referrals across all service lines.
- Cultivate and sustain relationships with potential referral sources.
- Raise community awareness of Partners In Care through ongoing education and outreach initiatives.
- Advocate for patients within the post-acute care spectrum and the broader community.
- Prioritize achieving targets for Hospice and Home Health services to meet census goals.
- Collaborate with the COO to devise and uphold an outreach plan aligning with agency objectives.
- Demonstrate and promote Partners In Care values authentically within the community.
- Assist the Health Information System team in fostering provider relationships for documentation collection.
- Oversee customer relationship management to improve outreach and territory management efficiency by maintaining regular routing, current territory knowledge, and prompt updates.
- Utilize data for setting targets, planning routes, defining goals, and crafting messaging.
- Develop and execute education and training programs covering clinical and non-clinical subjects for facility partners.
- Utilize Partners In Care service lines to educate primary care providers, clinical staff, and referral partners.
- Expand market presence by engaging with communities and healthcare leaders to provide education about Partners In Care services.
- Identify and leverage community platforms for educational events, such as health fairs and community groups.
- Establish and maintain positive relationships with facilities to ensure Partners In Care remains the primary provider of post-acute services.
- Identify opportunities for enhanced customer service and collaborate with clinical operations as needed.
- Engages with primary care and clinical providers to educate them about hospice and other service lines.
Required Skills & Qualifications:
- Bachelor’s degree or equivalent experience and at least one year’s experience in outreach, sales, and/or education.
- Ability to market and deal tactfully with customers and the community.
- Excellent observation, verbal, and written communication skills.
- Ability to work independently and collaboratively while navigating a high volume of internal and external tasks.
- Ability to pass a background test, drug screen, and TB test.
- Current automobile insurance, a valid Oregon driver’s license, and reliable transportation.

About 150 local volunteers work with Partners In Care in fundraising, resource coordination, and other areas of community support.
