Home Care Coordinator

We are seeking a motivated and compassionate individual to join our team as a Home Care Coordinator. In this role, you will be responsible for conducting quality assurance visits, facilitating introductions between caregivers and clients, and updating care plans.

Responsibilities:

· Conduct quality assurance visits to ensure the highest standard of care is being provided

· Facilitate introductions between caregivers and clients to create strong and trusting relationships

· Update care plans based on client needs and feedback

· Collaborate with caregivers, clients, and families to ensure seamless communication and coordination of care

Requirements:
