Diabetes Educator jobs in Indiana
Diabetes Educator educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Collects patient data to monitor health status and proactively manage the patient's condition. Being a Diabetes Educator facilitates skill development and behavior modification so that the patient is able to successfully manage the disease. Typically requires a bachelor's degree in nursing or equivalent. Additionally, Diabetes Educator typically required to be a Certified Care and Education Specialist (CDCES). Typically reports to a manager or head of a unit/department. The Diabetes Educator occasionally directed in several aspects of the work. Diabetes Educator Details
Essential Job Duties- Performs comprehensive learning needs assessment with regard to physical, psychological, and environmental factors that may affect the plan of care.
- Plan of care which includes goals and prioritized nursing/medical approaches derived from data collected.
- Utilizes additional resources in goal setting and meeting learning needs.
- Implements strategies to provide for patient's participation in goal setting, health maintenance and maximization of health capabilities.
- Notifies physician of significant changes in patient's condition and potential or suspected complications.
- Evaluates the patient for diabetes knowledge and status of diabetes management.
- Currently licensed by the Indiana State Board of Nurses' Registration and Nursing Education as a registered nurse with one year in diabetes education preferred.
- Certification by the American Association of Diabetes Educators (CDE) required or willing to obtain within 2 years of hire. Possible extension may be granted of up to 6 months to accommodate testing availability depending on hire
