Diabetes Educator educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Collects patient data to monitor health status and proactively manage the patient's condition. Being a Diabetes Educator facilitates skill development and behavior modification so that the patient is able to successfully manage the disease. Typically requires a bachelor's degree in nursing or equivalent. Additionally, Diabetes Educator typically required to be a Certified Care and Education Specialist (CDCES). Typically reports to a manager or head of a unit/department. The Diabetes Educator occasionally directed in several aspects of the work.     Job Responsibility

    • Plans and conducts nutritional programs, under supervision, to assist in the promotion of health and control of disease.
    • Assesses nutritional needs and considers patients' cultural, educational, and learning issues.
    • Provides individualized nutrition education to patients, their families, and/or significant others based on their needs assessment.
    • Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility.
    • Communicates patient information to referring providers and appropriate staff in a timely manner.
    • Documents nutrition information discussed, written materials provided, and evaluation of patient’s receptiveness and comprehension of the education provided.
    • Performs related duties as required. All responsibilities noted here are considered essential functions of the job under the Americans with Disabilities Act. Duties not mentioned here, but considered related are not essential functions.

    Job Qualification

    • Bachelor’s Degree in Food, Nutrition or Dietetics, required.
    • Registered Dietitian, required.
    • Current Diabetes Educator Certification, required.

                                        OR

    • Bachelor’s Degree in Nursing, required.
    • Current license to practice as a Registered Professional Nurse in New York State, required. 
    • Current Diabetes Educator Certification, required.
    • Minimum of three (3) years progressively responsible, related experience to ensure familiarity with scope of program.
