Registered Dietitian (Hybrid) Part-time
Location: Work from home with occasional travel
Salary Range: $28.00 to $30.00 (depending on experience) $0.55 mileage reimbursement
Summary:
- Performs all the duties of a RD Care Manager with a specialized population.
- This role possesses specific knowledge and experiences, which enable the case manager to better serve the patient, promoting patient health status.
- Provides and coordinates case management services that reflect the mission, values, philosophy, policies/procedures and standards of care that A&D Home Health Care is committed to uphold.
- Provides and documents patient encounters according to insurance standards.
- Communicates to and obtains pertinent information from the physician and their respective practices. The care manager maintains patient confidentiality.
- Works collaboratively within a multidisciplinary team model to promote care management services provided to each patient in collaboration with primary care practices.
Qualifications:
- Graduate of an accredited School of Dietetics.
- Currently licensed as a Registered Dietitian eligible to practice in the State of Michigan.
- Minimum of one-year experience as a Dietitian plus at least one year of practice Home Health Care.
- Prefer Case Management certification.
- Demonstrated knowledge of primary care practices and regulations.
- Positive, professional interpersonal skills.
- Meets health and malpractice requirements demanded of all clinical/field staff that provides service through the agency.
- Meets CEU requirements required by the State of Michigan for re-licensure.
Essential Job Functions and Physical Requirements:
- Able to interact successfully with patients, family members, medical professionals, hospitals, and other A&D employees.
- Must be able to communicate both verbally and in writing with superiors, physicians, field staff, office personnel, and patients/families.
- Have visual/hearing ability sufficient to comprehend written/verbal communication and perform accurate assessments.
- Able to see, hear and use a telephone without the use of extraordinary assistive devices.
- Must be able to operate an automobile and have access to personal transportation.
- Able to perform tasks involving physical activity, which may include heavy lifting and extensive bending and standing.
- Must be able to develop appropriate care plans based on a patient’s assessment findings.
- Must be able to complete multiple tasks, meet multiple deadlines and work within a demanding environment.
- Able to keep track of details including number of calls, progress with practice assignments, and specific practice requirements.
- Will provide advocacy for patient’s to secure appropriate services based on patient needs, choice, and program eligibility.
- Must be able to maintain patient documentation in a complete, coherent, and timely manner.
- Will provide social/emotional support and problem solving techniques to patient and care giving system.
- Able to report for work on a regularly scheduled basis.
- Must be able to successfully provide effective team support.
