Disease Management Case Manager jobs in Delaware
Disease Management Case Manager coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Acts as a liaison between patient/family, employer and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Disease Management Case Manager responsibilities include but are not limited to documenting case progress, identifying health risks, and reporting the findings of the case study at appropriate intervals. Requires an associate's degree/bachelor's degree, and is licensed to practice nursing. Additionally, Disease Management Case Manager typically reports to a supervisor or manager. Education/Experience: Bachelor’s or Master's in Social Work, Nursing, Behavioral Science, or equivalent experience. 2 years of related care management experience License/Certification: Valid driver’s license. RN (Registered Nurse) or licensed behavioral health clinician; LMSW or LCSW preferred DFH LTSS ONLY: Licenses listed above OR individuals with a Bachelor’s/Master’s degree in health, human, social work or education services with one or more years of qualifying experience; 2 years of related experience in case management with the aged, including management of behavioral health conditions, or persons with physical or developmental disabilities, or HIV/AIDS population DE Only: Home and site visits required Pay Range: $54,000.00 - $97,100.00 per year Centene offers a comprehensive benefits package including: competitive pay, health insurance, 401K and stock purchase plans, tuition reimbursement, paid time off plus holidays, and a flexible approach to work with remote, hybrid, field or office work schedules.
