Hospice Director directs, supervises, and governs the hospice program including inpatient care, home care and bereavement follow-up. Administers and maintains quality assurance, environmental, and infection control policies in accordance with facility objectives. Being a Hospice Director prepares reports for hospital administration on activities of the facility operation. Develops hospice related educational and informational programs. Additionally, Hospice Director requires a bachelor's degree of nursing. Typically reports to top management. The Hospice Director typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. Capable of resolving escalated issues arising from operations and requiring coordination with other departments. To be a Hospice Director typically requires 3+ years of managerial experience.     POSITION SUMMARY

    The Director of Psychosocial Services is responsible for supervising licensed and support field TEAMembers as well as directing Psychosocial Program services to include Social Work, Volunteer, Spiritual Care, and Bereavement services. This TEAMember works in collaboration with the Interdisciplinary team to meet the emotional and psychosocial needs of clients and families. This position is responsible for maintaining clinical and supervisory records in compliance with state licensure, federal standards and corporate policies.

    ESSENTIAL FUNCTIONS

    • Directs all daily Senior/client social service operations including providing direct oversight of the establishment and implementation of social service policies.
    • Ensures compliance with all state and federal social service regulatory requirements and the Social Work Practice Act.
    • Directs the implementation of improved work methods and procedures to ensure social services are delivered in accordance with policy.
    • Ensures seamless home care service delivery by providing direct oversights of coordination of care with other clinical service providers.
    • Ensures the timely and adequate delivery of Hospice Counseling services to Seniors/clients.
    • Supervises the assessment, planning, implementation, and evaluation of social service clinical care by members of the Psychosocial team.
    • Provides oversight and coordinates medical social services and activities including psychosocial, bereavement care, hospice counseling and case coordination. Ensures timely delivery of these services and making team assignments.
    • Assures proper documentation of social services in clinical records in compliance with state and federal standards. Problem solves computer documentation issues.
    • Actively participates as an Interdisciplinary team member, offering insight and suggestions for care plan updates for Seniors/clients.
    • Contributes to continuity of care and effective communication between Senior/client/family, Hospice TEAMembers, Home Health Agencies, Inpatient Facilities, Ancillary Services, and Physicians.
    • Monitors, coordinates, and evaluates Senior/client care given by assigned clinicians.
    • Participates in Senior/Client development activities such as community presentations, office calls and marketing presentations.
    • Hires and supervises Psychological TEAMembers in Lincoln and in regional offices as assigned.
    • Willingly takes on additional duties and/or assignments, assisting with projects and responding to requests as assigned or as necessary.


    AGE REQUIREMENT At least age 18.

    EDUCATION Graduate Masters from an accredited school of social work or licensed by the State of Nebraska as a Licensed Mental Health Practitioner (LMHP).

    EXPERIENCE One year of clinical experience appropriate to the counseling and case work needs of the terminally and/or medically ill required. Home Health Care, Nursing Home, or Hospice experience preferred. Training and experience in dealing with grief preferred.

    CREDENTIALS Must be certified by the State of Nebraska as a master’s level social worker. CPR required. All licenses/certifications must be kept up to date at the expense of the TEAMember. Valid Nebraska Driver’s license (unless otherwise allowed by law) and a good driving record required. Must possess auto insurance at or above Nebraska statutory requirements. Must meet Tabitha's vaccination requirements.
