    Your day as a Care Coordination Liaison

    As a Care Coordination Liaison at HME Specialists, you will play a critical role in ensuring a seamless transition for patients from hospital to home. Working closely with Account Representatives and Case Managers, you will expedite discharge processes and facilitate timely delivery of necessary medical equipment. Your responsibilities will include learning Brightree software, educating patients on equipment usage, obtaining required documentation, and delivering equipment to patients' homes. With a strong customer-centric focus, you will maintain stock and consignment closets, provide exceptional service, and uphold the company's values and mission.

    What we're looking for in a Care Coordination Liaison

    To excel as a Care Coordination Liaison at HME Specialists, candidates should possess a high school diploma or equivalent, with some related experience or training. Customer service experience in the Durable Medical Equipment field is highly valued, as it demonstrates the ability to provide exceptional care and support to patients. Basic healthcare knowledge, such as being an EMT, MA, or LPN, is advantageous in understanding medical terminology and equipment usage. Strong communication skills are essential for effectively educating patients and interacting with healthcare professionals.

    A problem-solving mindset, empathy, and attention to detail are key attributes for success in this role. The ability to adapt quickly, learn new software tools like Brightree, and work collaboratively in a fast-paced environment are also vital skills for this position.

    Knowledge and skills required for the position are:

    • High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience. Customer service experience in Durable Medical Equipment is a big plus.
    • Basic Healthcare knowledge - EMT, MA, LPN all a plus
