The Seattle Human Services Department (HSD) lives out its mission as one of the largest contributors to Seattle's safety net. As designated Area Agency on Aging (AAA) for King County, HSD’s Aging and Disability Services division (ADS) manages the continuum of care that supports older individuals and those living with a disability, and their families.

ADS is seeking a creative and collaborative leader to support our growth, team development, and unit operations in our Seattle program office. This position will work closely with ADS’s Renton office manager to manage the division’s Medicaid long-term care case management and care coordination programs, which include: Title XIX Core Services, Veterans Directed Services, Health Homes, Hospital to Home Care Transitions, Medicaid Transformation Demonstration Project, Family Caregiver and Respite services, and onsite support and coordination for Seattle Housing Authority senior buildings and supportive housing providers. The Manager will directly supervise 5-6 clinical supervisors and a senior counselor in the Seattle office and will be responsible for the work of approximately 60 staff.
This position will report to the Care Coordination Program Director. The Manager 2 will work in partnership with the ADS leadership team to advance the division’s strategic operations goals to elevate the experiences of Black, Indigenous, Asian, Pacific Islander, and Latinx people to ensure a healthy thriving workforce, a safe and collaborative working environment, and strong systems and partnerships to deliver quality services.
The Care Coordination Program Manager will
Minimum Qualifications:
Education: B.S. in Social Sciences, Human Resource Management, Business Administration, or a related field is required (or a combination of education and/or training and/or experience which provides an equivalent background required to perform the work of the class).
Experience: Five years of experience in human services or contract administration, service delivery, community organizing, and three years of supervisory experience preferably in the areas stated above.
OR: An equivalent combination of education and experience that demonstrates the ability to perform the position duties
Certification/License:
Current State of Washington driver's license or evidence of equivalent mobility.
