    The Seattle Human Services Department (HSD) lives out its mission as one of the largest contributors to Seattle's safety net. As designated Area Agency on Aging (AAA) for King County, HSD’s Aging and Disability Services division (ADS) manages the continuum of care that supports older individuals and those living with a disability, and their families.

    ADS is seeking a creative and collaborative leader to support our growth, team development, and unit operations in our Seattle program office. This position will work closely with ADS’s Renton office manager to manage the division’s Medicaid long-term care case management and care coordination programs, which include:  Title XIX Core Services, Veterans Directed Services, Health Homes, Hospital to Home Care Transitions, Medicaid Transformation Demonstration Project, Family Caregiver and Respite services, and onsite support and coordination for Seattle Housing Authority senior buildings and supportive housing providers.  The Manager will directly supervise 5-6 clinical supervisors and a senior counselor in the Seattle office and will be responsible for the work of approximately 60 staff.  

     

    This position will report to the Care Coordination Program Director. The Manager 2 will work in partnership with the ADS leadership team to advance the division’s strategic operations goals to elevate the experiences of Black, Indigenous, Asian, Pacific Islander, and Latinx people to ensure a healthy thriving workforce, a safe and collaborative working environment, and strong systems and partnerships to deliver quality services.  

     

     

    The Care Coordination Program Manager will

     

    • Support division commitment to lead with race, promote relational culture, and progress to anti-racist organization.
    • Co-manage the operations of direct and subcontracted long-term care case management and care coordination programs.
    • Directly coach, mentor, and supervise clinical team leaders and be responsible for work of over 60 program staff. 
    • Manage personnel and program resources, to include analyzing staffing needs, hiring, coaching, and labor relations.
    • Provide proactive leadership to foster staff morale and teamwork.
    • Demonstrate excellent written, oral and interpersonal communication skills including the ability to be a solution-oriented facilitator and program spokesperson.
    • Enhance customer service and client safety through staff training to ensure clinical competency, technological knowledge and adherence to program requirements and standards.
    • Provide managerial support and consultation on complex cases relative to mitigating risk and maintaining adherence to case management program policies and procedures.
    • Build relationships and collaborate with a diverse network of community partners, including aging and disability network service providers, governmental agencies, healthcare and behavioral health systems, and housing providers.  

     

    Minimum Qualifications:

     

    Education: B.S. in Social Sciences, Human Resource Management, Business Administration, or a related field is required (or a combination of education and/or training and/or experience which provides an equivalent background required to perform the work of the class).

     

    Experience:  Five years of experience in human services or contract administration, service delivery, community organizing, and three years of supervisory experience preferably in the areas stated above.  

     

    OR: An equivalent combination of education and experience that demonstrates the ability to perform the position duties

     

     Certification/License

    Current State of Washington driver's license or evidence of equivalent mobility.  


    Desired Qualifications:

    • Strong leadership and management experience demonstrating the ability to effectively manage teams during organizational transitions and program changes.  
    • Demonstrated ability to effectively oversee strategic initiatives to service-oriented and regulatory programs, and direct the work of supervisors and professional staff.
    • Knowledge of and experience with the administrative, contractual, and clinical requirements of long-term care case management and care coordination programs. 
    • Excellent written, oral, and interpersonal communication skills, including experience serving as program spokesperson and team facilitator.
    • Strong motivation, delegation, and decision-making skills.
    • Experience creating innovative solutions within complex operations and systems.
    • Strong experience as a team player and demonstrated ability to collaborate successfully with a wide variety of stakeholders, including community-based organizations, governmental agencies, healthcare and behavioral health systems, and housing providers.
    • Ability to work under pressure with multiple deadlines. 
    • Demonstrated knowledge of core business management principles (e.g., staffing, financial, resource allocation, relationships).
