5 Door Recovery, a Residential Alcohol and Drug Treatment program, has an immediate opening for an Intake Coordinator who acts as the face of 5 Door Recovery Monday through Friday 8:00am to 4:30pm. As the first person with whom both internal and external customers come in contact in person, over the phone and via email, the Intake Coordinator exemplifies the values of 5 Door Recovery. This position is responsible for screening potential clients, coordinating admissions with both the individuals and referral sources, and conducting intakes. Principle Accountabilities

Program Management

• Responds to all inquiries and provide referral information to potential clients, family members, counties, or other funding sources.
• Utilizes the American Society of Addiction Medicine (ASAM) criteria to ensure appropriateness of admissions or obtains said information from referral source.
• Coordinates funding authorizations for incoming clients with the counties of residence and private insurance carriers. Coordinates prior authorizations, co-pays, and deductibles.
• Maintains strong relationships with referral sources through prompt and professional communication and problem resolution. Informs the director of any issues or concerns related to our relationships with referral sources.
• Ensures regulatory compliance is maintained with intake procedures.
• Completes all file reviews and related PQI upon client discharge, insuring compliance with all related regulations and agency/program policies and established goals.

Client Services

• Responds to all inquiries and provide referral information to potential clients, family members, counties, or other funding sources.
• Responsible for orientation of new clients to the program expectations and guidelines, conducting facility tour, and making introductions to staff and counselor-of-record.
• Ensures that all admission procedures are completed thoroughly and according to regulations and program protocol.
• Conducts breathalyzer and UA tests of female clients as needed.
• Provides assistance and support for all clients as needed (examples include obtaining medications, transportation arrangements, checking-in belongs, etc.).

Administration and Support

• Maintains accurate daily census with admission and discharge dates, room, and counselor assignments.
• Trains and supports staff who answer phone line during non-business hours to ensure accurate information is provided to callers and that calls are handled with professionalism.
• Attends weekly Clinical Staffing to coordinate admissions, level of care status changes, and discharges.
• Other duties as needed or assigned.
