Licensed Practical Nurse - Home Care administers nursing care in a home care environment under the supervision of a registered nurse or other medical supervisor. Provides basic medical care, including changing bandages, administering medication, and collecting specimens. Being a Licensed Practical Nurse - Home Care ensures the health, comfort and safety of patients by assisting with bathing, feeding, and dressing. Monitors and reports changes in patient's condition to supervisor. Additionally, Licensed Practical Nurse - Home Care requires a high school diploma. Typically reports to a registered nurse or supervisor. Requires graduation from approved LPN educational program. Requires a state license to practice. Licensed Practical Nurse - Home Care's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. - The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. DUTIES & RESPONSIBILITIES Provides direct patient care as defined in State’s Nurse Practice Act. Implements plan of care initiated by the registered nurse. Provides accurate and timely documentation consistent with the plan of care. Collects assessment data and performs observation to report to and collaborate with RN(s) and physician(s). Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the registered nurse and/or Assistant Director of Nursing or Director of Nursing. Uses equipment and supplies effectively and efficiently. Participates in personal and professional growth and development. Performs other duties as assigned by the registered nurse.
