Recruits new physicians and manages the credentialing and orientation process. Qualifications
- Provisional MHRT-C (Required) or eligibility for licensure in Maine.
- Bachelor's degree in social work or a related field (preferred)
- Knowledge of community resources and systems of care for mental health
- Strong communication and interpersonal skills
- Ability to work both independently and collaboratively with interdisciplinary teams.
- Ability to manage a case load and meet required documentation standards.
- 1 year of case management experience (preferred)

Role Description
General Description of Responsibilities: The Adult Care Provider (ACP) provides services that focus on the specific needs of adults with behavioral health diagnoses and decreased ability to function independently in their homes and in the community, The ACP is responsible for developing the individual service plan to ensure that the services are approved, and will also work collaboratively with interdisciplinary teams, including psychiatrists, nurses, and other clinicians to meet client's needs by the best ways.
