Medical Social Worker (MSW) - Home Care provides social work services to home care patients and their families. Assesses and treats emotional and behavioral problems related to patient illness. Being a Medical Social Worker (MSW) - Home Care participates as a member of multi-disciplinary team and may assist patients in treatment planning. May contribute to the formulation of discharge plans. Additionally, Medical Social Worker (MSW) - Home Care requires a master's degree of social work. Typically reports to a senior case manager. Medical Social Worker (MSW) - Home Care's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. Position Summary:
Plan, organize and implement Medical Social Services within the confines of state licensing law to individuals, families and groups in accordance with agency policies, procedures, processes and agency code of ethics. This will be done in a manner consistent with efficiency, cost effectiveness and regulatory compliance.
Qualifications
- Master's Degree from a school of social work.
- One year of Master level social work in a health care setting required.
- One year of home health experience preferred.
- Competency in computer skills.
Special Requirements
- Access to transportation in the agency service area(s) with valid drivers license and auto liability insurance.
- Current CPR certification.
- Ability to perform duties with minimal supervision, manage multiple projects/changing priorities and employ critical thinking skills.
- Excellent communication, organizational and human relation skills.
- Registered, certified, licensed as required by state law for the profession.
