Minimum Data Set (MDS) Coordinator (LPN) jobs in Vermont

JOB DESCRIPTION:

Under the general direction of the Director of Nursing Services and in collaboration with the Nursing Unit Managers, is accountable for insuring the completion of a comprehensive assessment of resident’s needs and the development of a resident plan of care designed to meet resident needs. Promotes communication of resident’s status, condition changes and resident’s with the Nursing Unit Manager and other members of health care team. Functions within the limits of Vermont Nurse Practice Act, Code of Ethics for Nurses, Nursing Standards of Practice, Administrative and Nursing Department Policies and Procedures. Supports the philosophy, objectives and goals of CVMC/Woodridge and Nursing Department.

EDUCATION:

Graduation from an accredited Nursing Education Program with current RN licensure or compact licensure recognized by the State of Vermont is required.

EXPERIENCE:

5 years of MDS experience after 2019 required for this remote position

Three to five years of experience in a long-term care setting as a Team Leader or Charge Nurse.
