Job Overview
We are seeking a detail-oriented, data-driven MDS Coordinator for a 30 day contract assignment for a reputable Skilled Nursing Facility (SNF) near Long Island, NY. Travel, lodging, and meal reimbursement available if needed! Must be able to start immediately.
The MDS Coordinator will be responsible for ensuring accurate and timely completion of the Minimum Data Set (MDS) assessments and care plans for our patients.
Duties
- Conduct comprehensive assessments of patients' medical, physical, psychological, and social needs
- Collaborate with healthcare providers to develop individualized care plans
- Ensure compliance with state and federal regulations related to MDS assessments
- Review medical records and documentation for accuracy and completeness
- Coordinate with interdisciplinary teams to optimize patient care
- Utilize ICD-9 coding and Epic software for documentation purposes
- Perform utilization reviews to maximize patient outcomes
- Provide medical management support in various specialties such as ICU and pediatrics
Requirements
- At least 2 years of MDS Coordinator experience (preferably in a SNF setting)
- Availability to start immediately
- New York RN License
