Global Dynamics is currently seeking a Pharmacist to join our team.
Degree/Education: Bachelor's, Masters, or doctorate degree in pharmacy from an accredited training instituted by the Accreditation Council for Pharmacy Education (ACPE) Pharmacy College, or a Foreign Pharmacy Graduate Examination Committee (FPGEC) certificate.
Certifications : Basic Life Support
Experience: Minimum of 1 year of professional pharmacy experience after graduation. Must have outpatient experience.
Board Certification: None
Licensure/Registration: Maintain an active, valid, current, and unrestricted license (with no limitations, stipulations or pending adverse actions) to practice as a Pharmacist in any U.S. state/jurisdiction.
Duties : Core duties include but are not limited to the following:
- Perform a full range of pharmacist procedures IAW AF Pharmacy Manual.
- Deliver comprehensive pharmaceutical services within the personnel, supply, and equipment capabilities of the facility IAW AF Pharmacy Manual.
- Counsel patients regarding appropriate use of medications IAW AF Pharmacy Manual.
- Review patient profiles at the time of dispensing in order to closely monitor medication therapy.
- Ensure prescription directions for use, dose, medication interactions, therapeutic duplications or overlaps, allergy information, age specific dosage, and other pertinent data are appropriate for individual patients. Intervention with providers shall be made for noted discrepancy of above listed items IAW Defense Health Agency Procedural Instructions (DHA-PI) 6025.31, Military Medical Treatment Facility Pharmacy Operations.
- Compound medications as required IAW DHA-PI 6025.31.
- Ensure accuracy of all pharmaceutical products prepared or processed by technical staff IAW AF Pharmacy Manual.
- Supervise inventory control of all pharmaceuticals in the pharmacy, especially scheduled pharmaceuticals, and ensure pharmacy security. Maintain appropriate records and security of all scheduled controlled substances according to applicable regulations IAW AFI 44-102.
- Perform routine maintenance on pharmacy equipment IAW manufacturer recommendations.
- Document medical interactions and pharmacy interventions with prescribers, report medication errors and adverse drug reactions in accordance with local policy IAW AF Pharmacy Manual.
- Train clinical staff on new medications or guidelines as necessary.
- Ensure work areas are clean and safe according to applicable IAW The Joint Commission (TJC) Medication Management Standards.
- Dispense pharmacist-prescribed medicines as necessary IAW AFI 44-102.
- Answer phone calls.
- Monitor for drug interactions, over dosage and adverse reactions IAW DHA-PI 6025.31.
- Review of proper drug selection, sub-therapeutic dosages, over dosages, adverse drug reactions and drug interactions in the outpatient setting IAW DHA-PI 6025.31.
- Prepare and dispense pharmaceuticals for patients of all ages in the outpatient pharmacy setting IAW AF Pharmacy Manual.
- Evaluation, management, and consultation regarding drug therapy for patients for all ages with selected diagnoses by MTF IAW AF Pharmacy Manual.
- Recognize and ensure appropriate remediation of improper pharmaceutical therapy IAW DHA-PI 6025.31.
- Maintain appropriate records and security of all scheduled controlled substances according to applicable regulations IAW DHA-PI 6025.31.
- Shall always protect patient confidentiality and privacy as required by the Health Insurance Portability and Accountability Act (HIPAA) of 1996.
