Nurse Manager - Long-Term Care plans and implements the overall nursing policies, procedures, and services for a long-term care facility. Maintains nursing staff by recruiting, selecting, and training clinical staff. Being a Nurse Manager - Long-Term Care ensures that nurses are current in competencies, assessments, licensure, certifications, and other annual training. May provide direct patient care and is responsible for evaluating patient health. Additionally, Nurse Manager - Long-Term Care may require an advanced degree. Requires Registered Nurse (RN). Typically reports to a director. The Nurse Manager - Long-Term Care manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions.     Schedule (7p-7a) every other weekend.

    BASIC PURPOSE:

    The Shift Supervisor is responsible for all operational activities in the facility during the non-business hours, such as evenings, nights, and holidays. Provides clinical coordination, mentoring and quality assurance within the nursing services.

    MAJOR WORK ACTIVITIES:

    • Serve as administrative authority for the efficient and effective operation of all departments on assigned shift. Notifies Administrator, Department Director, Clinical Coordinator and/or Household Coordinator of urgent concerns or major changes during the shift.
    • Ensure adequate staffing through coordination of available resources, which include PRN, part-time and full-time nursing staff, and contract nursing personnel as available. Collaborates with nurse leadership.
    • Conduct rounds throughout the facility to monitor operations and the delivery of nursing care, visit residents and family members. Assess quality of care issues in each department on a continuous basis through monitoring resident care, documentation, investigation of abuse/neglect, grievances and family concerns.
    • Assure regulatory compliance by encouraging appropriate record keeping and charting. Initiate impromptu audits and observations to confirm competencies of household staff.
    • Guide the implementation of Clinical Policy & Procedure which ensures safe, sanitary and efficient practices supporting professional nursing care standards.
    • Provide guidance and assistance to all departments with policies and procedures.
    • Provide leadership for problem analysis, resolution and effective communication to facilitate quality service delivery to residents, families and employees.
    • Supervise personnel in other departments as needed with regard to the delivery of services to ensure a safe, clean and pleasant environment in the facility.
    • Provide input to Department Directors, Clinical Coordinators and Household Coordinators for evaluating performance of individual staff in a timely manner (30-60-90, annual, and as needed), and assist Department Directors, Clinical Coordinators and Household Coordinators with performance review conferences to include developing individual performance improvement plans with the employee and, when indicated, initiate disciplinary action including suspensions as needed.
    • Participate in various quality of care committees and activities.
    • Advocate for resident rights; educate residents, their families and staff regarding the rights of residents outlined under State and Federal guidelines.
    • Ensure compliance with company standards for quality, safety, cost control, policies and procedures.
    • Assist with proper completion and investigation of incident, injury, medication error and/or medication discrepancy reports.
    • Respond to emergency call as necessary.
    • Support management/administration decisions through periods of change or stress.
    • Responsible for completing accurate staffing roster and updating daily as needed.
    • Ensure appropriate staffing when call-ins are received.
    • Perform specific work duties and responsibilities as assigned by the Director of Nursing or Clinical Coordinators.

    • Clinical Skills:Clinical skills sufficient to meet all the primary responsibilities identified above; meets state licensing and all other appropriate regulations for position, as well as organization’s expectation of position clinically (direct nursing, teaching, supervision, organizational oversight). Completion of MDS-CAA’s and care planning. Must have baseline knowledge of federal and state regulations governing clinical care with a willingness to ongoing education as it relates to regulatory process.

Very strong observational and assessment skills with regards to physical and medical conditions and manifestations, coupled with ability to discern effective response, treatment and including pursuing additional expertise from RN or M.D.

    • Learning and Teaching Centered: Is able to model person-centered practices, as well as evaluate and provide direction and coaching to other staff as needed.

    • Conflict Facilitation: Able to mediate healthy conflict resolution between parties within household as need arises.

Able to provide clear directions and expectations as well as providing clear, effective feedback. Able to demonstrate and elicit mutual trust and respect. Able to communicate residents’ medical needs clearly with staff and families, facilitate other staff participation in medical care by eliciting others’ observations, suggestions, etc. Able to communicate clearly and effectively with providers.
    • Dementia Sensitivity: Able to teach and nurture care practices for persons with dementia that meet needs while protecting person-hood and sense of well-being.

    KNOWLEDGE, SKILLS AND WORKING CONDITIONS:

    • Ability to teach, delegate and supervise other personnel.
    • Knowledge of documentation requirements related to timeliness, accuracy and completeness.
    • Ability to disseminate appropriate information.
    • Demonstrate functional knowledge of business processes and operations.
    • Good communication skills, both verbal and written.
    • Ability to perform mathematical functions as they relate to pharmaceuticals and statistics.
    • Working knowledge of State and Federal regulations as they apply to the long term care setting
    • Ability to perform physical exertion at least 75% of the time (walking, standing, stooping, kneeling, bending).
    • Ability to perform intermittent lifting, pushing and carrying objects weighing between 30-40 pounds.
    • Moderate exposure to infectious diseases.
    • Exposure to minimal noise and varying degrees of stress.
    • Physical job demands may be subject to possible modifications to reasonably accommodate individuals with disabilities.

    MINIMUM QUALIFICATIONS:

    Education:
    Associate Degree in Business or Heath Care related field.

    Licensure/ Certification:
    Current North Carolina licensure as Licensed Practical Nurse or Registered Nurse.

    Experience:
    Minimum 1 year work related experience in business, leadership or health care.

    An equivalent combination of education and experience may be considered.
