Nurse Supervisor - Phone Triage supervises the day-to-day operations and coordinates activities of nursing teams in a nurse triage call center. Plans work schedules and monitors call levels to ensure sufficient staffing and adherence to protocols. SUMMARY OF JOB: A Triage RN receives and prioritizes all incoming calls in order to continue delivery of safe, holistic care to a population with multiple co-morbidities and advanced chronic illnesses. The Triage RN implements the nursing process during phone encounters to assess the patient and caregiver situations and provide appropriate support including education of the disease process to the patient and family. Triage team working during core business hours are facility-based positions.

RECRUITMENT REQUIREMENTS: Minimum of 2-3 year Med/Surg, acute care (hospital) and/or post-acute care nursing experience required; 1 year of home care experience preferred; BSN preferred; ER/Triage experience a plus; able to work weekend, holidays; able to demonstrate professional competencies in a home care setting; able to efficiently utilize technology (cell phone, computer).
