Job Summary:

We are seeking a dedicated Care Coordinator to join our team. The Care Coordinator will be responsible for facilitating patient care by providing administrative support and ensuring effective communication between patients and caregivers.

Responsibilities:

- Coordinate patient care.
- Maintain accurate and up-to-date patient records
- Communicate with caregivers, patients, and families to ensure seamless patient care
- Provide administrative support

Qualifications:

- Proficiency in medical terminology
- Experience in a medical setting

- Experience as an Assistant
- Strong organizational skills with attention to detail
- Ability to multitask and prioritize in a fast-paced environment
- Knowledge of medical administrative support functions

- Associates Degree is preferred.
