This RN Care Coordinator position supports the inpatient patients and families at MHealth Masonic Children's Hospital. It is primarily based in the NICU but will also support new mothers who may need discharge support services such as IV fluids, IV abx etc. The RN Care Coordinator role drives timely hospital transitions to home by removing barriers to discharge. Complex infants and pediatric patients may need support from the RNCC with things such as enteral feeds, IV antibiotics, durable medical equipment, arranging home ventilators and trach supplies, extended care nursing, coordination of outpatient specialty care and complex care handoffs to outpatient providers and speciality care coordinators.
This position will have a weekend call rotation which is approximately every 7th weekeend and one on-call holiday each calendar year.
AD in Nursing
5 years clinical experience
1 years working as a care coordinator/case manager
Active MN Registered Nurse license
Case Management Certification
BSN
Basic Life Support (BLS)
Past Care Coordination or bedside nursing experience a plus.

Have solid understanding of use of evidence based guidelines.
