    Summary :

    The patient navigator is responsible for assisting the organization with breast, lung, and colorectal cancer screening services. This includes direct client outreach and navigation discussing the importance of screenings, types of screening options, risk assessments, as well as facilitate scheduling. The Patient Navigator will connect patients with clinical, financial, and transportation resources as needed. The Patient Navigator will investigate delays in compliance with these screening services and assist patients and providers in removing barriers for improved outcomes and access to care. The patient navigator will work collaboratively with the access center, Women’s Health Center, imaging, and other internal stakeholders to assist in timely scheduling of screening procedures and to reduce delays in referral management. The patient navigator will ensure work aligns with and meets all grant guidelines and deliverables. The patient navigator will assist the imaging and women’s health departments with quality and data reporting.

    Responsibilities :

    • Provide outreach to patients past due for cancer screening with a focus on breast, lung, and colorectal screening services, providing education and appointment options.
    • Report grants related information in a timely manner to Imaging Director and Director of Community Health & Business Development who is the grant management leader.
    • Schedule imaging exams per department protocol in coordination with Access Center staff.
    • Manage outstanding orders and no-show reports, conducting proper follow up as needed with patient/provider/office staff.
    • Participate in community events promoting prevention and education programs.
    • Update spreadsheets, databases, and EMR as required to ensure appropriate follow-up and resolution.
    • Provide back-up coverage for the Women’s Health Center Representative position.
    • Other Duties as assigned by Director.
