PATIENT NAVIGATOR
*Additional $1.50/hr. for bilingual (English/Spanish)*

This is an excellent opportunity for a Patient Navigator (40 hrs/wk, benefits eligible).
PURPOSE of POSITION:
The role of the Patient Navigator is to remove patient’s barriers to care by identifying resources for patients, helping them navigate through health care services and systems and promoting patient health and well-being. The Patient Navigator position will provide comprehensive support services to assist patients who are receiving, seeking or eligible for additional care internally and/or externally.

- Ability to build relationships with different types of people including patients, organizations, and health care providers.
- Demonstrates organization and time management skills to manage a variety of tasks effectively.
- Ability to communicate clearly and effectively with patients, staff and other external parties in a courteous and friendly manner in person and over the phone.
- Experience in electronic health records (EHR), electronic practice management (EPM) system, Office 365, and Microsoft Office Products.
- Accuracy and attention to detail are essential.
- Effective written and verbal communication skills.
- Ability to read and implement written and verbal instructions.
- Ability to work independently and as a team member.
- Ability to handle sensitive information in a confidential manner.
- Bilingual English/Spanish skills desirable.
EDUCATION and/or EXPERIENCE:
1. High school diploma, GED or equivalent required. Bachelor’s degree preferred in related fields such as social work and/or counseling.
2. One (1)year related work experience providing patient navigation.
3. Experience in navigating insurances a plus.
PHYSICAL REQUIREMENTS:
Physical demands of the position necessitate hearing, talking, handling and grasping regularly. May require lifting up to 30 pounds. Specific vision conditions include close vision reading. Requires the ability to operate a computer, telephone, fax, copy machine and adding machine or calculator. Requires the ability to operate medical equipment.
WORKING CONDITIONS:
The position is generally performed in a medical environment but involves frequent interruptions. The noise level is usually moderate. The work environment characteristics are representative of those an employee encounters while performing the essential functions of this position. Evening and/or weekend hours may be necessary.
