    PATIENT NAVIGATOR

     *Additional $1.50/hr. for bilingual (English/Spanish)*

We serve over 60,000 patients annually in 22 health centers statewide, making us the largest and most trusted reproductive health care provider in Wisconsin.     This is an excellent opportunity for a Patient Navigator (40 hrs/wk, benefits eligible).


    PURPOSE of POSITION:   

     

    The role of the Patient Navigator is to remove patient’s barriers to care by identifying resources for patients, helping them navigate through health care services and systems and promoting patient health and well-being. The Patient Navigator position will provide comprehensive support services to assist patients who are receiving, seeking or eligible for additional care internally and/or externally.      

     

    QUALIFICATION REQUIREMENTS: 


    - Must be committed to providing excellent customer service and support the vision and values of Planned Parenthood of WI.

    - Must be sensitive to cultural and ethnic differences among staff and patients and be able to relate to diverse communities. 

    - Committed to providing non-judgmental, patient centered care to ensure the highest level of patient satisfaction.   

    - Ability to build relationships with different types of people including patients, organizations, and health care providers.  

    - Demonstrates organization and time management skills to manage a variety of tasks effectively.  

    - Ability to communicate clearly and effectively with patients, staff and other external parties in a courteous and friendly manner in person and over the phone.

    - Experience in electronic health records (EHR), electronic practice management (EPM) system, Office 365, and Microsoft Office Products. 

    - Accuracy and attention to detail are essential. 

    - Effective written and verbal communication skills. 

    - Ability to read and implement written and verbal instructions.   

    - Ability to work independently and as a team member.   

    - Ability to handle sensitive information in a confidential manner.   

    - Bilingual English/Spanish skills desirable. 


    EDUCATION and/or EXPERIENCE:   

    1. High school diploma, GED or equivalent required.  Bachelor’s degree preferred in related fields such as social work and/or counseling.  

    2. One (1)year related work experience providing patient navigation.  

    3. Experience in navigating insurances a plus. 


    PHYSICAL REQUIREMENTS:   

     

    Physical demands of the position necessitate hearing, talking, handling and grasping regularly.  May require lifting up to 30 pounds. Specific vision conditions include close vision reading. Requires the ability to operate a computer, telephone, fax, copy machine and adding machine or calculator.  Requires the ability to operate medical equipment.  


    WORKING CONDITIONS:   

     

    The position is generally performed in a medical environment but involves frequent interruptions.  The noise level is usually moderate.  The work environment characteristics are representative of those an employee encounters while performing the essential functions of this position.  Reasonable accommodations, however, may be made to enable individuals with disabilities to perform said essential functions. 

     

    Evening and/or weekend hours may be necessary.

    ESSENTIAL DUTIES and RESPONSIBILITIES
    • Initiate, build and maintain relationships with patients, offering support and guidance for medical and non-medical services that are available at PPWI and/or in the community. Facilitate patient referrals to outside providers. 
    • Monitor the patient navigation phone line and assure that patient calls are answered or returned in a timely manner as determined by PPWI. 
    •  Maintain documentation of all patient encounters per PPWI protocol and complete reporting requirements according to organization standards.  
    • Maintain and identify community resources for patients to overcome barriers to care for positive health outcomes. Remain aware of current services offered by service providers.  
    • Educate patients about their rights, insurance coverage, and payment options, and address any queries or complaints. Complete preauthorization for services with insurance providers. 
    • Provide financial screening for patients to determine eligibility for insurance programs or other funding sources to cover the cost of their services. 
    • Facilitate communication between the patient, PPWI providers and other external providers, managing health care records and needs, informing patients on financial aspects of care and linking patients to resources as needed.  
    • Provides information about PPWI’s services and obtains patient information.  Answers routine patient questions within scope of training. Directs patient concerns/questions that require further clinical assessment or medical advice to licensed personnel. 
    • Prepare reports related to utilization of financial assistance as needed to demonstrate compliance with eligibility requirements set by funders. 
    • Attend and represent the organization at professional conferences, in-service trainings, and meetings at the request of or with the approval of the supervisor. 
    • Performs other duties as assigned.
