Home Care Program Director (LPN) - Hiring Now in Atlanta

Position Overview

THIS IS NOT A REMOTE POSITION

We are seeking a qualified nurse coordinator to oversee our home care program. The ideal candidate will be responsible for coordinating and managing the delivery of home care services to patients, including assessment, planning, implementation, and evaluation. The nurse coordinator will also be tasked with managing a team of home care nurses and aides, ensuring compliance with regulations and standards, and collaborating with other healthcare professionals to provide comprehensive care to patients. Strong communication and organization skills are essential, along with a commitment to providing high-quality, compassionate care to patients in their homes.

As a supervisor, some of your duties include (but are not limited to):

• Overseeing in-home staff and LPNs
• Managing resident funds and schedules
• Coordinating with agencies
• Ensuring home maintenance
• Managing residents' medications
• Maintaining records
• Ensuring adherance to relevant regulations and policies
• Purchasing supplies
• Monitoring staff documentation
• Ensuring food is purchased according to meal plans
• Travel between residential care facilities and private homecare clients as needed utilizing company expensed vehicle

Schedule: Full Time (40 hours), Monday - Friday

Position Requirements

General:

• Associate's Degree in Nursing
• Current BLS certification
• At least 4 years of LPN experience, with atleast 1 year in Private Duty/Home care experience preferred
• Nurse Management (2 years)
• Care Coordination (1 year)
• Experience and comfort working with: G-Tube / TracheotomyTube/ Ventilator
• Basic Computer and Microsoft Suite Knowledge
• Effective time-management
• Interpersonal skills and ability to work with multiple personality styles
