Rehabilitation Services Supervisor - Home Care supervises the delivery of occupational therapy, physical therapy, and speech/language pathology programs to patients in a homecare setting. Responsibilities:
- Provide direct care to patients in their home, assisting with activities of daily living
- Assist with personal hygiene, including bathing, grooming, and dressing
- Monitor and report any changes or concerns to the appropriate healthcare provider
- Assist with mobility and transfers, including safely lifting and positioning patients
- Perform light housekeeping duties, such as cleaning and laundry
- Prepare meals and assist with feeding if necessary
- Provide companionship and emotional support to patients
- Follow care plans and maintain accurate medical documentation
Qualifications:
- Valid certification as a Home Health Aide or equivalent training/experience
- Previous experience in home healthcare or a related field is preferred
- Strong communication skills, both verbal and written
- Compassionate and patient-centered approach to caregiving
- Ability to work independently and follow instructions accurately
- Flexibility to adapt to changing schedules or patient needs
