Duties:
- Coordinate and manage care services for individuals in need of support
- Develop and implement care plans based on individual needs and goals
- Provide direct support to individuals, including assistance with daily activities such as personal hygiene, meal preparation, and medication management
- Utilize behavior management techniques to promote positive behaviors and reduce challenging behaviors
- Collaborate with other professionals, such as social workers and healthcare providers, to ensure comprehensive care for individuals
- Maintain accurate documentation of care provided and progress made by individuals
- Monitor and evaluate the effectiveness of care plans and make necessary adjustments
Experience:
- Previous experience in a similar role, such as direct support professional or care coordinator, is preferred
- Knowledge of behavior management techniques and systems is highly desirable
- Familiarity with home care services and regulations is a plus
- Excellent communication and interpersonal skills are essential for effective collaboration with individuals, families, and other professionals
- Ability to lift and assist with the mobility of individuals as needed
- Valid driver's license and clean driving record
