    Characteristics of the position:
     
    Under the general supervision of the Aging Director of the Buffalo Trace ADD, the ADRC Coordinator is responsible for the day-to-day functions and overall management of the Aging and Disability Resource Center (ADRC). This includes providing citizens with a trusted place for gaining information and assistance related to services and support for individuals of all ages but especially those who are aged or those with disabilities and their caregivers.
     
    ESSENTIAL JOB FUNCTIONS:
     
    • Demonstrate knowledge and understanding of the Aging and Disability Resource Center as well as extensive        knowledge of benefits and eligibility determination.
    • Answering the Aging and Disability Resource Center phone line.
    • Oversee the data information systems used by ADRC staff.
    • Document all contacts and respective assessment tools in the SAMS Mediware I & R system for client tracking within the necessary DAIL provided timeframe(s). 
    • Provide information, referrals, and assistance to both public and private-pay individuals about services and supports for which they are potentially eligible through home visits, office meetings, by telephone, or other methods of contact.
    • Provide benefits counseling to access public assistance services and advocacy to individuals aged eighteen (18) and older, persons with developmental and or intellectual disabilities, physical disability, mental illness and or substance use disorders, and for youth who are transitioning into the adult long-term care system.
    • Options Counseling, person centered needs assessments, referrals, preliminary care planning and short-term follow-up are an expectation for this position to help consumers needing long term support based on their needs and preferences.
    • Use creativity and critical thinking skills to find options, providing the inquirer with several options, offering information, assistance to encourage consumer directed decision making.
    • Provide specific information on eligibility and the process to apply for services and resources.  If appropriate, assists individuals in connecting with a resource.  As needed, will research additional or alternative resources.
    • Complete DAIL approved assessment tools appropriately and complete.
    • Assist potentially eligible Medicaid waiver consumers and/or their representatives with the submission of the Medicaid Waiver Management Application waiver application and guidance through the waiver process.   
    • Establish and maintain an ongoing process to evaluate customer satisfaction with ADRC services and involve stakeholders, consumers, state agencies, providers, and public and private partners in planning, implementing, and evaluating the ADRC.
    • Maintain professional relationships with other agencies and providers as a reciprocal referral system to ensure service delivery ease. 
    • Participate in program development, marketing and outreach activities including representing the ADRC and Area Agency on Aging and Independent Living at public information fairs. 
    • Establish and promote the Aging & Disability Resource Center as a single point of entry to publicly funded programs.
    • Manage and update respective wait lists for state and federally funded in-house programs.
    • Ensure the Aging & Disability Resource Center hotline is adequately covered during normal business operating hours, excluding agency approved official closings.  
    • Maintain State Health Insurance Program counselor status; Provide SHIP counseling and enrollment guidance as needed to callers; Provide referrals to SHIP Coordinator as deemed necessary. 
    • Produce reports and data as required.
    • Provide outreach and target services to those with disabilities and their caregivers as funding is made available. 
    • Assist in conducting public forum, consumer satisfaction surveys, needs assessments, etc., which can be used in the ongoing design and evaluation of the program.
    • Coordinate program activities with the Area Agency on Aging and Independent Living services.
     
    OTHER JOB RESPONSIBILITIES:
    • Complete DAIL required training. 
    • Job development trainings and staff meetings.
    • Assist with special projects and duties as assigned.
      
    SKILLS KNOWLEDGE AND ABILITIES:
    • Ability to effectively communicate with the public via telephone.
    • Knowledge of community resources.
    • Problem solving skills.
    • Good organizing skills to ensure prompt follow-up with calls and data entry.
    • Strong computer data entry skills
     
    EDUCATION, EXPERIENCE AND SKILLS REQUIRED:
    • Bachelor’s degree in human services field such as social work, counseling, public health planning or public policy. A bachelor’s degree with experience working with the elderly and disabled.
    • Experience with computer database systems.
    • Must possess a valid driver’s license.
