Responsibilities also include involvement in the discharge planning process. Additionally, Social Worker (MSW) requires a master's degree. Typically reports to a manager. Social Worker - MSW Qualifications:- Licensed in the state of Minnesota as an LGSW or LISW (Master's-Level Social Worker)
- At least one year of clinical Social Work experience
- Experience with Electronic Medical Records (EMR) preferred
- Make Social Work home visits to assess patients' psychosocial status
- Provide Social Work services in the home, including short-term individual counseling, community resource planning, and crisis intervention
- Work closely with all members of the health care team in development of the plan of treatment
