Staff Nurse - RN - Home Care Admissions evaluates patients referred to home care services and determines the types of services to be provided. Interviews the patient and family to determine appropriateness for services. Being a Staff Nurse - RN - Home Care Admissions obtains initial and ongoing referral documentation from physicians, insurers, and providers. Coordinates the development of patient home care plans that specify the frequency and duration of the services provided. Additionally, Staff Nurse - RN - Home Care Admissions initiates care with service providers. Completes admission, referral, and insurance forms. Utilizes health record management software. Typically requires a bachelor's degree in nursing or equivalent. Requires an RN license. Typically reports to a manager or head of a unit/department. We are a licensed nursing facility offering skilled care to Medicare, Medicaid and Private pay residents. West Ridge provides 24-hour nursing care services through registered nurses, licensed practical nurses, and certified nursing assistants. Nursing care for all levels of need can be met by West Ridge.
Education & Credentials
RN graduate from an accredited school of nursing required. Current licensure as an RN in the State of Iowa by the Iowa Board of Nursing, without restrictions, is required. CPR certification required. Failure to maintain current license and certification without restrictions will result in termination. May not be a debarred provider. Applicants must have geriatric experience and certification.

10. Complete health assessments on all residents at time of admission
11. Analyze and code diagnoses, operations and procedures from health records by using appropriate classification systems and entering data into computer system to process, store and retrieve resident health information.
12. Willing to work flexible hours to meet requirements of the job.
13. Meet all submission timelines of reports/records/documentation as required by state and federal rules, regulations and laws for health care facilities.
14. Monitor accrediting/surveying/regulatory body criteria to maintain compliance.
15. Participate in continuing education to maintain credentials and increase expertise and knowledge in professional clinical nursing and administration.
16. Keep current in the field of nursing through attendance at meetings, conferences, conventions and workshops pertaining to all aspects of resident care and restoration/ rehabilitation; maintaining membership in professional organizations.
17. Ability and willingness to perform RN duties whenever and wherever required.
18. On-call for emergencies that other supervisory personnel cannot address.
