Staff Nurse - RN - Home Care Admissions jobs in South Carolina

Providence Care is now hiring an Admissions Registered Nurse (RN) within the York, Chester and Lancaster counties in South Carolina for our Home Health Division.

Monday - Friday days 8:00 a.m. to 5:00 p.m. with strong work life balance!

Responsibilities:

• Performs a comprehensive assessment to include OASIS (Outcome and Assessment Information Set) evaluation of the home situation and medical history to determine what health teaching will be required and services needed.
• Develops and updates the plan of care in consultation with the physician and all clinicians who will be involved in the patient's home care.
• Re evaluates nursing needs and evaluates the outcomes of care.

Qualifications:

• Unencumbered RN license in the state you are applying required
• At least 1 year of nursing experience required
• At least 1 year of Home Health experience highly preferred
• Med/Surge experience highly preferred
• Wound care, IV, plurex catheter and or colostomy experience required
• Up to date CPR (BLS)
• Reliable transportation, auto insurance and driver's license required
• Ability and willingness to travel to patients homes, assisted living and/or nursing facilities within the territory
• Self Directed, organized, compassionate and reliable
