Job Summary: The Registered Nurse (RN) working in a skilled rehab nursing home/hospital is responsible for providing comprehensive nursing care to residents in a rehabilitative and long-term care setting. The RN will collaborate with the interdisciplinary healthcare team to assess, plan, implement, and evaluate individualized care plans. Duties and Responsibilities:
Resident Care:Conduct thorough assessments of residents' physical, psychological, and social needs.
Develop, implement, and update individualized care plans based on the assessment findings.
Administer medications, treatments, and nursing interventions as prescribed.
Monitor residents' progress, document observations, and adjust care plans accordingly.
Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility.
Provide emotional support and education to residents and their families regarding their conditions, treatments, and care plans.
Collaborative Approach:Collaborate with a multidisciplinary team, including physicians, therapists, social workers, and nutritionists, to coordinate comprehensive care for residents.
Participate in care conferences and contribute to developing and revising care plans.
