  • POSITION SUMMARY: This position reports to the CSM (Client Services Manager) in accordance with Company policy and regulatory requirements. This position is synonymous with the positions of Companion, Housekeeper, Homemaker, and Chore Worker.

    PRINCIPAL DUTIES AND RESPONSIBILITIES:

    1. Is knowledgeable of, interprets and supports the philosophy, purpose, mission, goals, policies and standards of the organization.

    2. Represents the company in a positive manner while creating an environment that fosters respect and courtesy for clients and other employees alike. Works within the company programs to promote safety and the well-being of all clients and employees.

    3. Provide general assistance and care to patient/clients in accordance with the non-personal care plan.

    4. Plans meals, purchases food and household supplies and prepares and serves nutritious meals in accordance with the non-personal care plan.

    5. Works to provide for a clean, safe, and orderly environment for the patient/client and family.

    6. Washes dishes, cleans the kitchen after meals, launders clothes, bed linens, and performs other housekeeping activities normally accomplished on a daily or weekly basis, in accordance with the non-personal care plan.

    7. Provides companionship for the patient/client as required.

    8. Medication reminders

    9. Performs other duties, such as transportation of the independent patient/client, when directed by the supervisor.

    10. Accurately and thoroughly documents services provided in accordance with agency policy on day services are rendered, submits in a timely manner.

    11. Immediately reports any changes in patient/client’s needs, condition, or incidents to the supervisor in a timely manner.

    12. Establishes and maintains effective communication and a professional relationship with patient/clients/families and co-workers participating in case conference as appropriate.

    13. Maintains and enhances skills through attending appropriate staff development programs.

    14. Appearance is professional and complies with agency dress code.

    15. Assignments are fulfilled with reliability and punctuality.

    16. Maintains a cooperative manner towards patient/client/family members of the healthcare team.

    17. Listens and responds to patient/client’s needs in a sensitive and timely manner either through own initiative as appropriate, or by referral to the supervisor.

    18. Assists the supervisor by providing feedback in regard to updating the non-personal care plan.

    PROFESSIONAL CONDUCT & DEVELOPMENT

    1. Participates in Quality Assessment and Improvement activities as assigned. Communicates information that enables collection and root cause analysis of data to identify opportunities for improvement.

    2. Attends, participates in and/or conducts internal staff development programs, obtains continuing education as required by Company policy, regulation.

    3. Maintains and enhances clinical practice skills.

    4. Performs other duties as assigned in response to patient/client, office and/or facility needs.

    5. Maintains strict confidentiality of patient and employee information.

    6. Maintains strict confidentiality of Agency proprietary and business information.

    OTHER DUTIES AND RESPONSIBILITIES:

    1. Reports for work on time and prepared to assume duties.

    2. Adheres to Agency dress code.

    3. Records work accurately and legibly.

    4. Follows policy for reporting in sick or absent.

    5. Schedules all time off with supervisory approval.

    6. Varies work schedules to meet Agency needs.

    7. Adheres to Agency policy, procedure and processes.

    8. Responds to internal and external customers in a professional and friendly manner.

    9. Offers assistance as needed or directs individual to appropriate resources with proper follow through.

    10. Wears ID badge in a visible location, if required.

    11. Complies timely with all requirements related to risk management, safety, infection control, TB screening, security and fire, as appropriate.

    12. Provides for a safe environment for patients, employees and others.

    13. Assumes responsibility for all mandatory in-services, certification, and/or licensure, competencies, etc.

    PREREQUISITES:

    A. Education:

    • High school graduation or GED required.
    • The candidate must have successfully completed a training program if required by regulation or contract.

    B. Direct Previous Experience:

    • Two years’ experience in a similar position desired.
    • Two years’ experience working with the elderly desired.

    C. Skills/Competencies:

    • Ability to read, write, speak and understand English fluently
    • Ability to work with minimal or close supervision.

    • Must conduct all business with a professional manner and with a high level of confidentiality. • Must pass competency testing on hire and on-going for the basics of, but not limited to:

    o Meal preparation, housekeeping, infection control, safety in the home, proper handling of emergencies, transportation, dog walking and care

    D. Other Qualifications:

    1. Effective January 1st, 2018 must have an Android or iPhone device (see Aide visit/documentation requirements)

    2. Current listing with no substantial finding on state Aide Registry, as state appropriate, if applicable

    Abilities:

    • Work indoors for the majority of the work day (fluorescent lights, HVAC system, carpeted/vinyl flooring, normal housing noise levels).
    • Provide a reliable means of transportation as required.
    • Possess the physical ability to perform job-related duties which may require lifting, standing, bending, transferring, stooping, stretching, walking, pushing, pulling and the ability to provide partial or complete assistance with non-personal activities of daily living, without assistance from another healthcare worker, or significant other.
    • Ability to work in time-sensitive or stressful situations.
    • Sit in a chair/stand for extended periods.

    Travel:

    • Up to 40% by land.

    Working conditions:

    • 50-90% of work is performed in a client’s home. The position does have exposure to malodorous, infectious, body fluids and some minimal exposure to noxious smells from cleaning/disinfectant agents.

    Eligibility to drive on company business and a satisfactory driving record required.
