We are currently looking for a full-time coordinator to become a part of our progressive team. In this role, the successful candidate possesses strong decision-making skills, excellent communication, and time management skills. The coordinator must work directly with patients and caregivers to support the daily operation of the agency.

Duties and responsibilities:

• Scheduling, maintaining, and assigning Home Health Aides to meet patients’ day-to-day needs.
• Ensure services are provided as prescribed by the client’s Plan of Care.
• Communicate with patients and aides to ensure they are satisfied with the care provided.
• Maintains patient’s files and health status with the patient through bi-weekly progress calls.
• Have the knowledge to work on Prebilling and Call Dashboard daily.

Qualifications:

• High School Diploma or equivalent required.
• Preferred one year of Home Care Case Coordination experience.
• Knowledge of HHA Exchange is highly preferred.
• Proficient in Microsoft Office (Outlook, Word, Excel).
• Must be organized and detail-oriented with excellent listening, written, and oral communication skills.
• Self-motivated, able to work independently and with teams, multitask, and make sound decisions when needed.
• Bilingual Russian/English. Other languages are a plus!

Job Type: Full-time

Pay: $20.00 - $25.00 per hour

Expected hours: No less than 40 per week

Schedule:

• 8 hour shift
• Monday to Friday
• Weekends as needed
