Wellness Program Consultant assists an organization in designing wellness programs that promote healthy employee behaviors. Helps develop and implement wellness initiatives that educate and encourage employees on achieving personal health goals. Being a Wellness Program Consultant analyzes the effectiveness and utilization of wellness programs and recommends changes to improve outcomes and employee satisfaction. Typically requires a bachelor's degree in area of specialty or in a related area. Additionally, Wellness Program Consultant typically reports to a supervisor or manager. The Wellness Program Consultant work is generally independent and collaborative in nature. Contributes to moderately complex aspects of a project. LPN Wellness Consultant
  • Aging With Grace, Aging In Place--The Health Club for Seniors
  • Lexington, KY FULL_TIME
  • Purpose: Assess and reassess each member’s health and create a plan of care for the members attending the clubhouse with RN Wellness Consultant's help. Keep member records according to 910 KAR 1: 160. Supervise nurse aide and volunteers concerning knowledge of, and medications and treatments, for members. Maintain emergency supplies and procedures and to train nurse aide and volunteers accordingly. Help everyone, including self, to best possible health.

    Qualifications for Wellness Consultant: Currently licensed and has a minimum of three years experience working with the elderly. Good communication and people skills required. Must show compassion and servant leadership. Must have and maintain a professional demeanor.

    • Does initial assessment with RN Wellness Consultant's help.. Creates plans of care to place in Wellness Maps. Communicates with each member’s physician and gets approval for plan of care and obtains orders for member from physician. Updates plan of care once per week a. Maintains a daily log of personal care services and treatments needed by each Member. Accomplishes plan of care or assigns another staff to accomplish and is responsible for making sure tasks are accomplished.
    • Works with other staff and volunteers to accomplish services while cheerfully encouraging member’s highest level of self-help.
    • Under RN Wellness Consultant's direction, creates and executes drills for emergencies and plans for disaster and fire. Keeps emergency kits stocked.
    • Trains nurse aide and volunteers in emergency procedures and follows a written plan accessible to all staff members.
    • Schedules and conducts fire and tornado drills. Performs checks on fire extinguishers.
    • Oversees Medicaid compliance for Medicaid eligible members.
    • Purchases (keeping receipts for proof of purchase or for reimbursement) as needed and inventories supplies for medical needs monthly. Orders and maintains nursing supplies, keeps track of budget, gives receipts to administrator monthly.
    • Oversees procedure for providing first aid and transporting members to a physician or hospital in case of emergency.
    • Follows prescribed procedures for admission, evaluation of needs, and discharge of a member.
    • Oversees infection control policy, including educating and monitoring nurse aide.
    • Ensures that the club’s medication policy is followed. Each member or the member’s caregiver checks in the medication in your sight each time they enter the club, and must check it out in your sight when leaving the club each day, or in sight of another qualified staff member. Only a licensed nurse is allowed to handle and dispense medication.
    • Follows policies and procedures for member records. Keeps member records in order and easily obtainable for other staff.
    • Takes weekly vitals on full time (6 hours per day, 4 or more days per week) members and monthly vitals on part time members. Weighs each member monthly.
    • Ensures that notes are made daily, in the Journal for documenting I & O’s, activities, and any applicable medical progress report.
    • Promulgates training material to nurse aide and volunteers and tracks compliance.
    • Gives all Aging With Grace potential employees drug screenings and keeps records of results.
    • Wears approved attire: casual dress with lab jacket or nice scrubs. No sandals, excessive jewelry or revealing attire, please.

    Job Type: Full-time

    Pay: $22.00 - $28.00 per hour
